TITLE: Contract Amendment No. 5 to Contract No. 2016-03, 2015/2016 Road Bond and Road Rehabilitation Project with Ajax Paving Industries of Florida, LLC in the amount of $213,203.05 plus $21,320.30 for contingency for a total of $234,523.35.

Background Information
On November 23, 2015, Contract # 2016-03 for the 2015-2016 Road Bond and Road Rehabilitation Project was awarded to Ajax Paving Industries of Florida, LLC. While the project has been under construction, staff has determined the existing base and asphalt on North Port Boulevard, from South Biscayne Drive to Chancellor Boulevard, is beginning to deteriorate and needs repaired. The repair work will also include the replacement of two aging stormwater cross drains under the road.
Ajax Paving Industries of Florida, LLC is currently mobilized in the area and is able to make the repairs. Staff is requesting $213,203.05 plus $21,320.30 for contingency to repair the road. This change increases the routine maintenance contract portion of $5,808,297.55 by $213,203.05 to $6,021,500.60. The following provides an explanation for the changes requested in Contract Amendment No. 5:
* Time Extension-Deletion: The contractor has requested additional 40 days to schedule the base replacement and overlay of North Port Boulevard from South Biscayne Drive to Chancellor Boulevard which will result in the completion date of April 13, 2017.
* Over/Underrun Contract Items: The proposed work on North Port Boulevard will increase the quantity of asphalt and road crossing pipes at original unit cost contract prices.
