TITLE: Interagency Agreement between the Sarasota Family Youth and Family Services (YMCA) and the City of North Port
Recommended Action
Approve an Interagency Agreement between the Sarasota Family Youth and Family Services (YMCA) and the City of North Port.
Background Information
Both parties desire to prevent at risk adolescents from entering the juvenile justice and child welfare systems through the use of community-based services and desire to facilitate and encourage communications between each other in order to better serve the citizens of North Port.
The parties agree to work together when making referral, reviewing eligibility criteria, using the referral process provided, and working cooperatively with staff to clarify acceptable referrals; include all information concerning referrals between the parties; maintain regular contact to assure proper coordination of services; provide verbal or written information that a client has kept his/her appointment, has been accepted for service, and is receiving services when provided with appropriate HIPAA compliant and medical release signed by client’s legal guardian; and if either party becomes dissatisfied with the referral process, services rendered, or information offered, both parties agree to meet to reconcile the differences.
This agreement has been reviewed and approved for legal content and form.
