TITLE: Approve Fourth Amendment Contract No. 2022-26 Water Pipeline Bridge Replacements - Haberland/Woodhaven/N. Toledo Blade for Change in Scope and Time Extension with Spectrum Underground, Inc., for an Additional Amount of $33,335.00, for a Total Contract Price of $821,201.35.

Approve Fourth Amendment Contract No. 2022-26 Water Pipeline Bridge Replacements - Haberland/Woodhaven/N. Toledo Blade for Change in Scope and Time Extension with Spectrum Underground, Inc., for an additional amount of $33,335.00, for a total contract price of $821,201.35.
Background Information
> On July 12, 2022, Commission approved Construction Contract No. 2022-26 - Water Pipeline Bridge Replacements for the replacement of the water pipelines affixed to bridge crossings along Haberland Blvd. Woodhaven Rd. and N. Toledo Blade Blvd.
> On November 22, 2022, Commission approved Amendment No. 1 to Contract 2022-26, which added the Oritz Blvd bridge watermain to the scope of the project.
> On February 28, 2023, Commission approved Amendment No. 2, which added to the scope of the project the replacement of the wastewater pipeline at the MacCaughey Waterway bridge crossing along Price Blvd, which is necessary due to the Price Blvd widening project moving forward.
> On April 11, 2023, Commission approved Amendment No. 3, which corrected an error on the bid form for the MacCaughey Waterway bridge crossing as well as relocate the bore location at the Ortiz Blvd watermain bridge crossing.
Due to the depth of the waterway crossing, the bore location for the forcemain being installed under the MacCaughey Waterway needs to be relocated to safely achieve the bending radius requirement for the HDPE pipe. This Fourth Amendment will relocate the bore location 100 linear feet to the west of the original path and add 30 days to the contract time.
