Recommended Action
Approve the Tenant Lease Agreement between the City of North Port and Centerplace Health, Inc., for suite B104 at the Family Service Center located at 6919 Outreach Way.
Background Information
Centerplace Health, Inc., a Florida Not-For-Profit Corporation, has requested to lease suite B104 at the Family Service Center located at 6919 Outreach Way.
This Agreement will be one (1) year ("Initial Term") that, unless otherwise terminated, will automatically renew from year-to-year thereafter, for no more than four (4) consecutive terms. Rent will be calculated in the amount of nine dollars and fifty cents ($9.50) per square foot, for a total monthly rental amount of one- hundred five dollars and twenty-nine cents ($105.29).
Centerplace Health's mission is to provide efficient and effective delivery of quality, affordable health services to a diverse population in a compassionate and caring manner. Centerplace Health provides dental, primary, and integrated behavioral health care services to Sarasota County residents at six (6) different locations.
The Tenant Lease Agreement with Centerplace Health, Inc., for suite B104 at the Family Service Center located at 6919 Outreach Way has been reviewed by the City Attorney and is legally correct as to form.
