TITLE: Approve Utilization of City of Clearwater, Florida Contract Between City of Clearwater and Life Extension Clinics, Inc. (“Life Scan”) RFP #53-23 Employment Related Health Care Services for Annual, Pre-Employment, and Fit-for-Duty Evaluations, in an Amount Not to Exceed $150,000 Annually Through the Term of the Contract Ending December 31, 2026, and Authorizing the City Manager to Approve any Modifications or Terminations in Concurrence With the City of Clearwater, Florida.
Recommended Action
Option 1: Approve utilization of City of Clearwater, Florida Contract between City of Clearwater and Life Extension Clinics, Inc. (“Life Scan”) RFP #53-23 Employment Related Health Care Services for annual, pre-employment, and fit-for-duty evaluations, in an amount not to exceed $150,000 annually through the term of the Contract ending December 31, 2026, and authorizing the City Manager to approve any modifications or terminations in concurrence with the City of Clearwater, Florida.

Option 1: Approve utilization of City of Clearwater, Florida Contract between City of Clearwater and Life Extension Clinics, Inc. • Pros: Allows the City to piggyback on an existing competitively procured Contract, reducing administrative time and ensuring timely access to required employee physicals. Life Scan provides comprehensive evaluations in accordance with National Fire Protection Association (NFPA) 1582: Standard on Comprehensive Occupational Medical Program for Fire Departments, ensuring compliance with industry standards. Additionally, continuity with an established provider supports consistent medical evaluations and familiarity with Fire Rescue operational requirements.
• Cons: The use of a Cooperative Contract reflects terms and conditions established by another agency.
Option 2: Deny utilization of City of Clearwater Contract RFP #53-23 with Life Extension Clinics, Inc. (“Life Scan”).
• Pros: Provides the City the opportunity to reassess its approach to employee physical examination services and consider alternative procurement methods.
• Cons: Requires additional time and administrative effort to complete a competitive procurement process, which may delay required employee physicals and impact compliance with NFPA standards, potentially affecting the readiness and safety of Fire Rescue personnel.
Background Information
The City of North Port Fire Rescue Department requires employee physical examinations, including annual, pre-employment, and fit-for-duty evaluations, to ensure personnel are medically capable of performing essential job functions. These evaluations are conducted in accordance with NFPA 1582: Standard on Comprehensive Occupational Medical Program for Fire Departments, which establishes medical requirements to ensure the health and safety of firefighters and the public.
The Collective Bargaining Agreement with the Suncoast Professional Firefighters and Paramedics Local No. 2546, International Association of Fire Fighters (IAFF) requires that the City provide annual physicals in accordance with NFPA 1582. Compliance with NFPA 1582 is critical to maintaining a safe and effective workforce, reducing the risk of on-duty injuries or medical events, and ensuring personnel are fit to perform the physically demanding and hazardous duties associated with fire rescue services.
The City seeks to utilize City of Clearwater Contract RFP #53-23 with Life Extension Clinics, Inc. (“Life Scan”), which was competitively procured. This cooperative purchasing arrangement allows the City to obtain these required services under the same terms and conditions without conducting a separate solicitation.
The City has reviewed the Contract and determined that its terms, pricing, and service provisions are in the best interest of the City.
The Contract between City of Clearwater and Life Extension Clinics, Inc. (“Life Scan”) RFP #53-23 Employment Related Health Care Services has been reviewed by the City Attorney and is legally correct as to form.

Financial Impact
Funding in the amount of $121,620 is budgeted in the North Port Fire Rescue Department Fiscal Year 2026 budget in account numbers 001-2222-526.31-04 and 110-2222-522.31-04. Any purchases made on or after October 1, 2026, or each year thereafter, against this Contract are contingent on future appropriated funds.
Procurement
The utilization of this Cooperative Contract is consistent with Chapter 2, Administration, Article VIII, Procurement, Section 2-403 (4) (a) (Exemptions) of the Code of the City of North Port, Florida, which permits the use of competitively awarded Contracts from other governmental entities.
The North Port Fire Rescue Department will utilize the established internal procurement process for all purchases under this Contract to ensure continued administrative oversight and compliance with the City procurement policies and procedures.
